Caregiver Support: 
 
Family members and friends are a vital part of a participant’s care team. CEI social workers, home care providers and other care team members work with family caregivers on a regular basis to make sure participants are safe and healthy in their living situations.
We provide many resources to caregivers:
 
  • Help with transportation of participants to and from medical appointments and the PACE day center
     
  •  Safe and secure day care for their loved ones
     
  • Direct contact with CEI care team providers and representatives for questions and concerns about their loved one’s condition

  • Referrals to community services and caregiver support groups
 
Call CEI outreach department (510-433-1150) if you know of a group of people (your place of worship, your book group, caregiver support group) who could benefit from a presentation about CEI.
