Your Center for Elders’ Independence (CEI) Interdisciplinary Care team is composed of a primary care physician, PACE Center manager, registered nurses, physical, occupational and recreational therapists, social worker, dietitian, your home care geriatric aide, PACE Center aides, and transportation representative.

As needed, your senior care services team mobilizes other services including medical specialists, laboratory and other diagnostic tests, hospital and nursing home care. When you first join CEI, you will receive an in-depth assessment from all of your CEI care team members. These PACE eligibility assessments will be made every six months or when there is a change of condition or a request for services. You and your caregiver(s) will continue to play an important role in managing your health and independence.

Continuous monitoring and frequent coordination allow the CEI care team to successfully manage your health and social conditions so you can stay as independent as possible and live in a place you call home.

Because the CEI care team members are in frequent contact with you, they can detect even the most subtle change in condition and modify your treatment plan. The care team has complete flexibility in tailoring a care plan for your needs so you can remain as independent as possible.
