Thank you for contracting with the Center for Elders’ Independence (CEI) and working to better the life of frail seniors in our local community. With your help, most CEI participants are able to remain in their homes and out of institutional settings, greatly improving the quality of their lives and those of their families.
The materials in this section of this website provide the information you need regarding referral authorizations and requests for additional visits/services, reporting your findings and recommendations, filing claims forms for PACE (Program of All-Inclusive Care for the Elderly), and getting reimbursed on a timely basis. We’ve kept our processes as simple as possible. Our intent is that you and your office staff, as dedicated PACE medical center geriatric providers, spend time treating our participants and interacting with our medical team rather than on administrative make-work.
If you need to call CEI for any reason, you will reach a human being rather than a machine 24 hours a day, 7 days a week, 365 days a year. Our hope is that you will find working with CEI convenient and efficient.

The Clinical Contact List includes the names and phone numbers for CEI’s Primary Care Physicians and Nurse Practitioners, who direct and oversee all care for participants, and information for our Behavioral Health and Transportation Departments.
The Administrative Contact List tells you who to call for contracting, credentialing, claims, appeals, utilization management and other functions. CEI is a small organization and you’re welcome to contact our management team directly.
